At Alexander Devine Children’s Hospice Service, we are deeply committed to ensuring that every young person and their family experience a smooth, empowering, and compassionate journey into adulthood. Our Transition Service guides young people through a structured pathway that bridges children’s and adult care.
Collaboration is at the heart of our approach: our Transition Lead works closely with adult hospices, healthcare providers, education, social care, and other teams through joint MDTs, visits, and shared planning to ensure continuity and confidence.

Transition Training Offer
Empower your staff to confidently care for young people moving into adult services. Our bespoke training covers clinical skills, holistic care, ethical considerations, and communication strategies, ensuring staff feel prepared, supported, and capable. Transition can be complex, but with professionals working together, it doesn’t have to be daunting.

Our Poster
As more young people with complex, life-limiting conditions reach adulthood, adult hospice teams face growing pressure to deliver developmentally appropriate, holistic care, often without specific training. Building the Bridge showcases an innovative collaboration between Alexander Devine Children’s Hospice Service and Thames Hospice that addressed this challenge. Through a co-designed education programme, adult hospice staff gained hands-on experience in complex medical care, ethical decision-making, and communication methods, transforming uncertainty into confidence.
This project didn’t just enhance knowledge; it built lasting collaboration across services, ensuring young adults continue to receive compassionate, skilled care as they move from child to adult hospice support.
